This effect is currently and has been for years published in Mark Wilson's Complete Course in Magic. It appears on page 32 as "The Fantastic Five" and is a damn good effect. Personally, I use it as an immediate follow up to "Panic"...Another damn good effect.

In all fairness this plot is probably a whole lot older than Mark Wilson or the other contributers to his excellent publication.
